A parent or legal guardian must be present during the athlete’s evaluation.
A licensed physician will review your health history and conduct a basic physical exam to determine if you are able to safely participate in your sport. Bring any necessary forms required by your school and glasses or contacts if you wear them.
Forms must be completed in their entirety prior to the patient being registered.
Minors require a parent or legal guardian present at time of visit. If the athlete is under the age of 18 & does not have a parent or legal guardian present, PUCBWS will not treat the patient. A parent must be present to discuss previous medical history & make sure follow care/instructions are addressed per the doctor’s recommendation. Any additional lab tests or services will result in additional charges. Please note that we do not offer “certificates of immunization” (these copies should be obtained from the administering healthcare provider). Offer not valid for enrollees in a government funded medical insurance program.
