Students that want to participate in school sponsored athletics may be required to have a Sports Physical. Visit your nearest WellStreet Urgent Care facility today for a FREE certified sports physical. Just another way WellStreet helps you to get well on your way!
• Just walk in, no appointment necessary
• A licensed physician will perform an examination
— Review of your health history
— A simple physical exam to determine if you are able to safely participate in your sport
• We will stamp the required medical forms and provide you with a summary of the exam
What to know before your visit
Wear comfortable clothes and bring any necessary forms. If required, a urinalysis can be performed which will result in additional lab charges. Please note that we do not offer “certificates of immunization” (these copies should be obtained from the administering healthcare provider).
